Prior to performing any exterior modifications to your home or lot, you must complete, sign, date, and return the Design Review Form (and any required attachments) to: Continental Country Club, Architectural Control Committee, 2380 N. Oakmont Dr., Flagstaff, AZ 86004.

If you are applying for more than one change, pay only the greater fee.

This form is provided for your protection. Please complete only those portions relating to the type of work you wish to begin on your property. The Architectural Control Committee (ACC) shall have 30 days from the date the completed form is received to evaluate and respond to this request, by phone and/or in writing.

Any work performed prior to approval from the ACC shall be subject to fines, and Continental Country Club reserves the right to require restoration of the property to its prior condition. Any alterations to this request must be resubmitted. If any work other than that which is indicated on this form is performed, approval will be voided and fines may be imposed; restoration of the property to its prior condition may be required as well. The ACC reserves the right to inspect the work upon completion. City permits must be obtained prior to work commencement for additions, remodels, and new constructions. All work must be completed within the allotted time frame.
